Additional Information:

• You may update your roster and add players for coverage up to 15 days from the start date of the season, by going back to the online link, and entering the additional #/s. Please provide the updated roster with the additional payment.

• The number of players must match the payment submitted.

• When a player has a claim, please provide them the claim form and claim contact information provided for any insurance questions

• The league will need to validate the player was on the roster submitted, payment was made, and the accident/injury occurred during the covered activity.
